In addition to the forms below, we will need a copy of your child's birth certificate and shot record. You will also need to pick up and complete an information card to be kept on file.  A STUDENT WILL NOT BE CONSIDERED OFFICIALLY REGISTERED UNTIL ALL FORMS AND REGISTRATION FEE ARE RECEIVED.
